Criteria of surgical indication of CTguided stereotactic evacuation of intracerebral haematoma were advocated based on the author's clinical experience. Ninety patients with ICH were treated with the Iseki frame and Archimedes type of haem atoma drill from January 1982 to May 1986. The patients were composed of61 males and 29 females aged between 42 and 87 years (Tables 1-3). Fifty-nine patients had haematoma in the putamen (Table 4) and 31 patients in the thalamus (Table 5). The haematoma extended into the lateral ventricles in 21 patients, but the rest of the group (69 patients) had no ventricular haemorrhage (Tables 6 and 7).

6 ml of autologous blood were respectively injected during 30 minutes in small fractions with 10 minutes interval. The total volume of blood injected was sufficient to produce a sustained and permanent increase of the intracranial pressure. The possible beneficial effect of open surgery, stereotactic evacuation, urokinase infusion and both latter techniques combined was tested in different groups of 4 dogs, having subcortical or basal 46 L. : Stereotactic Evacuation and Local Urokinase Administration in Intracerebral Haematomas ganglia haematomas, and were compared with a control group of untreated animals, 10 in each location, to observe the natural history of the process.

Local urokinase plus stereotactic aspiration showed a high efficacy in controlling delayed basal ganglia and subcortical blood collections. Other therapeutic approaches behaved almost as the natural history. Keywords: Intracerebral haematoma; urokinase; stereotactic surgery; intracranial pressure. Introduction Optimal treatment for patients presenting with spontaneous intracerebral haematoma remains undetermined and controversial. Conservative conduct4 as well as open surgery8 are certainly indicated in several cases.
